肥厚型心肌病 ( hypertrophic cardiomyopathy；HCM ) 是貓最常見的心臟疾病之一。本研究利用心臟超音波檢查台北市地區的家貓 ( n = 245 )，以了解台北市貓隻罹患HCM的盛行率，並探討其發生的相關因子。結果顯示，受檢的貓隻中有32隻被診斷為HCM，盛行率為13.1％。雄性、年齡4歲以上或體重5公斤以上者有較高的風險，其勝算比 ( odds ratio；OR ) 依序為3.04、2.78、5.01。在品種上，純種貓有較高的風險 ( OR 6.40 )，且以波斯貓與美國短毛貓為好發品種 ( OR分別為2.88、4.68 )。在聽診時發現有心雜音亦為重要的危險因子 ( OR 35.75 )。此外，本研究亦從受檢貓隻中篩選出正常對照組，以建立心臟超音波檢查參數的正常參考值範圍。在HCM的病例中，發現左心室肥厚的型式皆為節段型肥厚，多發生在心室中隔處。發生二尖瓣收縮期前移 ( systolic anterior motion；SAM ) 者有10例，其中有9例出現動力學性左心室流出道的狹窄；在color-flow Doppler下，SAM病例的二尖瓣逆流噴流的形態與無SAM者不同。由本研究結果得知，在台北市有相當比例的貓隻罹患HCM但並無明顯臨床症狀，對這些病例而言存在著一個潛伏性的威脅，值得臨床獸醫師加以重視。
In order to realize the prevalence of HCM in domestic cats in Taipei City, 245 cats enrolled in this study and examined by various examinations, then following by echocardiography for diagnosing HCM. Of the 245 cats, 32 were diagnosed with HCM, and the prevalence was 13.1%. Risk factors were male (OR 3.04), age order than four years (OR 2.78), and body weight more than 5 kg (OR 5.01). Pure breed cats were at significantly greater risk (OR 6.4), and Persian and American Shorthair were two high risk breeds (OR 2.88 and 4.68 respectively). Cardiac murmur was also an important risk factor in physical examination findings (OR 35.75). Moreover, we established reference ranges of echocardiographic parameters from the control group in this study. In the HCM group, the patterns of left ventricular hypertrophy were segmental, dynamic left ventricular outflow tract obstruction was identified in nine of ten cats with mitral systolic anterior motion (SAM), and the mitral regurgitant jets were typically eccentric in the cats with SAM. There were perspective cats with HCM but subclinical symptom in Taipei. It is a potential risk for those cats, and is worth of pay attention to this problem for the clinical veterinarian.
